ADMINISTERING MEDICATION(S) TO
STUDENTS
With
the increased number of students needing to take medications, it is now
required by law for schools to have a written policy governing the administration
of drugs and prescription drugs to students.
The practitioner prescribing the medication has the power to
direct, supervise, decide, inspect and oversee the administration of said
medication. In order to ensure this
power, the following criteria will be required prior to the administration of
medication in the Port Edwards Schools:
I.
Medications
include any form of prescription or non-prescription drugs (over-the-counter).
II.
Non-prescription medications
must be supplied by the pupil’s parent or guardian in the original sealed
manufacturer’s package. The package must
list the ingredients and recommended therapeutic dose. A pupil may be administered a
non-prescription drug in a dosage other than the recommended therapeutic dose
only with the written approval of the pupil’s practitioner.
III. Medication
to be given in school must have:
a.
Written order from the prescribing practitioner.
b. Written parent/legal guardian permission.
c. Properly
labeled container with the following:
1. Child’s name
2. Name of drug and dosage of drug
3. Time to be given
4. Prescribing practitioner’s
name
5. Prescription number (when a prescription drug
is ordered)
IV.
Medication(s) will be taken by
students at a designated time supervised by authorized personnel who have
received appropriate training that has been approved by the Wisconsin
Department of Public Instruction.
V.
It is the responsibility of the student,
if appropriate, and not school personnel, to get his/her medication at the
designated
time.
VI. Limited quantities of medication should be
kept at school.
VII. All
medication to be administered by school personnel at school
will be stored safely in a locked drawer, cabinet or file.
VIII. Parents/guardians
must notify the school when any change in drugs, dosage and/or time occurs or
if medication is renewed. Any change in
drug, dosage, time or renewal of medication must have a new practitioner’s order and a new parent/guardian
consent form.
IX. Newly
written orders for medication from practitioners
must be renewed annually for students on long-term medication(s).
X.
Non-prescription medication
(over-the-counter) will not require a practitioner’s
order form unless deemed advisable by appropriate school personnel. They will require all other provisions as
with prescription medication(s). Verbal
permission may be acceptable from parents/guardians, providing written consent
follows when students request “as needed” medication, i.e., Tylenol, antacid.
XI.
Students taking medication on an
“as needed” basis or taking prescription medication without dispensing by
authorized school personnel are doing so by their own and/or their
parents’/guardians’ own responsibility.
XII.
An accurate record-keeping system for each pupil receiving medication
will be established by the school nurse and secretaries.
XIII. An asthmatic student may possess and use a
metered dose inhaler or dry powder inhaler while in school, at a
school-sponsored activity or under the supervision of a school authority if all
of the following are true:
a)
the student uses
the inhaler before exercise to prevent the onset of asthmatic symptoms or uses
the inhaler to alleviate asthmatic symptoms;
b)
the student has the
written approval of the student’s practitioner and , if the student is a minor,
the written approval of the student’s parent/guardian; and,
c)
the student has
provided the school nurse with a copy of the required approval or approvals.
XIV. It is strongly recommended that
parents/guardians bring medication(s) to avoid broken containers and/or to
prevent
other individuals from ingesting the medication(s).
XV. School personnel authorized to administer
medication to students shall be provided appropriate instruction.

PROCEDURE
FOR ADMINISTERING/DISPENSING MEDICATION IN SCHOOL
I. Medication
will include all forms of drugs (i.e. prescription or non-prescription pills,
liquids, ointments, creams, cough drops, lozenges, vitamins, drops for eyes or
ears for oral, topical or injectable use).
II. A. No
medication(s) will be administered by school personnel without a practitioner’s order for medication administration form being filled out and returned to authorized
school personnel.
1. The practitioner’s
order for medication administration form will ensure that the practitioner retains the power to direct, supervise,
decide, inspect, and oversee the administration of medication.
2. The practitioner’s
order for medication administration form will include signed, written
instruction(s) for the administration of the prescribed medication(s).
3. The practitioner’s order for medication administration
form will include a signed, written statement which identifies specific
conditions/circumstances under which contact should be made with him/her in
relation to conditions or reactions of the student receiving the medication(s).
4. No medication(s) brought to school for
administration will be dispensed without the practitioner’s
order for medication administration form.
In the absence of this order, a request for practitioner’s
order must be sent to the prescribing practitioner’s,
along with the practitioner’s order for
medication administration form, to be filled out and signed by the prescribing practitioner and returned to authorized school
personnel.
B. No
medication(s) will be administered by authorized school personnel without a
parent/guardian consent form being filled out, signed and returned to authorized school personnel.
1. The parent/guardian consent form will
include authorization for school personnel to give medication(s) as prescribed
by the practitioner.
2. The
parent/guardian consent form will include authorization for school personnel to
contact the practitioner directly as needed.
3. If
verbal permission by parent/guardian is deemed acceptable, a written consent
form must follow.
C. Medication will not be dispensed/administered in school without being received in a
proper container, with the following information printed on the label:
1. Student’s
name
2. Name of drug/dosage
3. Time
to be given
4. Practitioner’s name (applies to prescription and
non-prescription drugs)
5. Prescription
number (applies to prescription drugs)
III.
Medication(s) will be
administered by the school nurse or by other individuals who have been
identified to do so. If the student
refuses the medication(s) or the time of administration needs to be altered,
parents/guardians will be informed.
IV.
Students, when of appropriate age
and development, will be responsible for reporting to the individual who is
administering the medication(s). When
the student does not report, medication(s) will be taken to the student
whenever possible. If the student does
not receive the medication(s), parents/guardians will be informed.
V.
Only necessary, limited
quantities of medication(s) are to be kept at school.
A. If
medication is to be given for a specific number of days, only that amount
of medication(s) will be accepted.
B. If
medication is on a PRN basis (whenever or as needed), only limited quantities
will be accepted.
C. If
medication is on a daily basis for the school year, only limited quantities will
be accepted and empty bottles will be sent home for refills.
D. If
medication is discontinued, parents/guardians will be requested to pick up
remaining medication(s) at school.
E. Except as otherwise provided under Section XIII, all medication(s) remaining at school at the end of
the school year must be picked up by parents, after being informed by school
personnel. Remaining medication(s) will
be destroyed.
VI.
All medication(s) to be administered by school personnel will be
kept in a safe place not accessible to students (i.e., locked drawer, cabinet, file) and checked out only by designated, authorized
personnel who will administer the medication(s).
VII.
The length of time a medication is to be administered, times to be
administered and dosage must be in the written instructions from the
prescribing practitioner. Any change from the original instructions or
renewal of medication(s) shall be reported by parents and a new order from the
prescribing practitioner and a new
parental/guardian consent form must be received
by the school prior to the administration of medication(s).
VIII. The length of time for which a
medication is to be administered is not to exceed a current school year. New prescriptions and parental/guardian
consent forms must be received annually for any pupil’s yearly medication(s).
IX.
Non-prescription
(over-the-counter) drugs will be administered only with parental/guardian
approval, unless a practitioner’s order form is also
deemed advisable by appropriate school personnel, as is indicated by written consent on the parent/guardian consent
form.
Non-prescription
medication(s) must be labeled, as are prescription medication(s), with:
a. Student’s
name
b. Name
of medication/dosage
c. Time
of administration
d. Practitioner’s name
X.
a. Students
taking medication(s) without consent, approval or knowledge of the school are
doing so by their own responsibility and/or their parents/guardians own
responsibility.
b. If a
student takes medication(s) without dispensing by authorized school personnel,
the prescribing practitioner must indicate on the practitioner’s
order form that the student may take medication(s) at school without authorized
school personnel dispensing the medication(s), when a prescription drug is
being taken. All other information must
be included by the prescribing practitioner as
listed on the practitioner’s order form.
c. A parent/guardian consent form must indicate
that the student is to take medication(s) without the authorized school
personnel dispensing the medication(s).
All other information must be included by the parent/guardian as listed
on the parent/guardian consent form. This
applies to prescription and non-prescription drugs.
XI.
When a student requests a
non-prescription (over-the-counter) drug (i.e., Tylenol, throat lozenges) and a
parent/guardian form has not been received by the school, verbal permission
from the parent/guardian may be obtained by authorized school personnel,
providing written consent follows.
XII.
a. A list of students needing medication(s) during school hours and
those students authorized by practitioners and
parents/guardians to take their own medication(s) will be maintained and kept in the appropriate health
services offices. This list will
include:
1. Student’s
name
2. Name
of medication(s)
3. Grade
level/homeroom
4. Dosage
5. Route
(oral, topical, injectable)
6. Time
of administration
7. Duration
of medication(s)
8. Reason/diagnosis
9. Practitioner’s name
10. Comments
to be made
This
list will be updated as needed.
b.
An
individual record for each student receiving medication(s) will be kept. This record will include:
1. Student’s
name
2. Name
of medication
3. Dosage
4. Prescription
number (applies to prescription drugs)
5. Time
of administration
6. Duration
of medication
7. Route
(oral, topical, injectable)
8. Diagnosis
9. Practitioner’s name
10. Practitioner’s phone number
11. Initials/name
of authorized school personnel dispensing medication
Attached
to this record will be the parent/guardian consent form and the practitioner’s order form. This record will be kept in the appropriate
school area.
c. Appropriate
school personnel will be advised of medication(s) a student is receiving, along with side
effects and expected results, and will be asked to report any unusual behavior
the student on medication may display.
Parents/guardians
and the prescribing practitioner may request
medical/medication information be kept confidential. This request will be honored.
XIII. The
school cannot be responsible for any accident/injury/lost medication that
results from a student transporting medication(s) to and/or from school. If a student must transport medication,
parents must give written permission when the parent/guardian consent is
signed.
Medication(s)
will not be sent home with students unless written permission is on file.
This
permission will release the school from any responsibility of any
accident/injury or lost medication(s) that results from the student
transporting the medication(s) to and/or from school.
